My wife's results from her lumpectomy showed angiolymphatic invasion and perinueral invasion. They couldn't find a sentinal node and she didn't have the aux node dissection so they don't know the status of those. She's doing Chemo/Radiation/Tamoxifen anyway.

My question is:

Is there really a difference between the angiolymphatic/perinueral invasion and having positive nodes? They know that microscopic cancer cells have escaped the tumor into all of the above vessels, but isn't that what they're checking for in the lymph nodes?

For all intents and purposes, isn't this really the same?

I guess I feel that the invasion(s) are really being downplayed, when I do research that shows it to be a huge factor.

Any thoughts on this? Anyone here? Dr Leo?

I can't answer your question...but I did have positive nodes 3+/14. And I'm starting chemo now and that is to be followed up with radiation and Tamoxifan. Someone here will be able to answer better than me but just know there is a lot of support out there.

I can't answer your questions, either. Sorry. I had axillary nodes removed and they were negative.

You might try www.imaginis.com for information. Dr. Leo will probably respond. He will probably have more insight. You should take your questions to your wife's surgeon and/or oncologist. Welcome to the board. Hope you find the help you are looking for here. In the meantime, best wishes to your wife in her treatment. Lauri

Hello

Even though these are not the same as positive lymphnodes, they mean that the tumor is agressive and has gained access to the lymphatic and bloodstream. So it is definitely important that she gets the most aggressive therapy, and it seems that she is already getting that.

best regards,
Leo

I'm going to put my 2 cents in as a question........well 2 to be exact.... lol

Shouldn't the nodes be removed either before or after chemo? I know some have chemo before surgery but they still had them removed.

Second question. My pathology report showed unknown vascular invasion because my surgeon said that the blood vessels/veins or whatever they checked had no cancer cells in them, but I had 9+/16 nodes. I was told that wasn't a good test and that just because they didn't see any, didn't mean it wasn't in my blood system. Is there a chance that it was contained in my breast and nodes and didn't get into my blood stream? I was treated aggressively because of the stage and my age and the nodes, but I've not read anyone else's path that didn't find it in the blood stream.
Thanks
nancy
